WHAT IS COMMUNITY LIVING AND SUPPORT?
Community Living and Support is a one on one service that supports the person with living successfully in their home and/or community. Staff will support the person by learning new skills and developing individualized activities to improve target areas and preferences. Staff will support the person with increasing-maintaining life skills; maximize his or her self-sufficiency; increase self-determination and enhance the person's opportunity to have full membership in his/her community. Areas of skill acquisition are: interpersonal, independent living, community living, self-care, and self-determination.
Solid Foundation also offers Community Living Facilities and Supports. CLFS is a comprehensive community living support benefit for eligible IDD adults. There are five levels of CLFS ranging to help accommodate recipients who reside in facilities, in his/her own home, with parents/guardians/family members and/or other paid staff arrangements. CLFS provides specialized and generic training, treatment and integrated health or related services, directed toward helping the member function with as much self-determination and independence as possible. CLFS is a comprehensive community living support benefit for eligible IDD adults with Medicaid.
